Upper and lower eyelid surgery with attention to eye shape and support

Blepharoplasty can improve excess upper-eyelid skin, lower-eyelid bags and selected contour changes around the eyes. Planning should preserve eyelid closure, support and the natural relationship between the eyelids, brows and cheeks.

Upper eyelid assessment

The examination considers skin excess, fat distribution, brow position, previous surgery, dry-eye symptoms and eyelid function. Brow descent can contribute to apparent upper-eyelid heaviness and may require a different or combined plan.

Lower eyelid assessment

Lower-eyelid bags, skin quality, lid support, tear-trough contour and cheek volume are evaluated. Surgery may involve fat repositioning or removal, skin treatment, support procedures or fat grafting depending on anatomy.

Who may be suitable

Candidates should have concerns that match the available surgical correction and health appropriate for elective surgery. Dry eye, thyroid eye disease, previous eyelid operations, contact-lens problems, visual symptoms and medications must be disclosed. An ophthalmic assessment may be recommended when indicated.

Recovery and limitations

Bruising, swelling, temporary dryness, watering, light sensitivity and tightness are common early symptoms. Fine asymmetries may remain because the two eyes and brows are naturally different. Lower-eyelid swelling and contour settling can continue beyond the initial social-recovery period.

Risks

Possible complications include bleeding, infection, delayed healing, dry-eye symptoms, visible scarring, asymmetry, eyelid retraction or malposition, difficulty closing the eye, persistent swelling, double vision and rare visual complications. Careful assessment and conservative tissue management are important, but no operation is risk-free.
